Different agencies handle different issues. Starting with the agency that has authority over the issue can reduce delays and duplicate filings.

Use the official SOMB complaint form for an alleged standards violation by an individually listed SOMB provider. The official form accepts anonymous complaints and requires enough information to process the complaint.
Use Colorado DPO for a complaint involving a professional, occupation, or business regulated by a DPO board or program. DPO accepts online or paper complaints and explains the limits of its licensing authority.
The Office of Community Corrections directs people to use the facility chain of command first, then the local county community corrections board, and then the state office if the issue remains unresolved.
The Colorado Victim Rights Act complaint process is separate from provider discipline. Its official page explains eligibility, local resolution, privacy limits, and the formal review process.
Registration and registry-record questions are handled by the Colorado Bureau of Investigation and are not the same as a complaint about a SOMB-approved provider.
Agency forms, contacts, deadlines, and jurisdiction can change. A filing with one agency does not automatically file with another agency or extend a legal deadline.
